Liability Waiver and Media Release Agreement

As a participant, parent or legal guardian, you are reading this document as a participant of New Jersey Gray Wolves FC in that you are registering yourself or another individual to participate in New Jersey Gray Wolves FC activities. You do so having the legal authority to enter into such an agreement.

By reading this agreement you agree that you have read and understand the New Jersey Gray Wolves FC Liability Waiver (hereinafter “Waiver”) and waive New Jersey Gray Wolves FC (NJGW) from all liability relating to injuries that may occur before, after and during participation of any activity on location. You agree to hold NJGW completely free from any and all liability, including but not limited to financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

Through registration and participation, you forfeit all right to bring a suit against NJGW in connection with participation in NJGW. Players will make every effort to obey safety precautions as listed in writing and as explained verbally. Participant and or player will ask for clarification when needed. Participant and or player must inform the club of any pre-existing or recent injuries that the player may have prior to participating in any NJGW activities. If any precautions or medications are need to participate it is the responsibility of the participant and player to inform, and if necessary, educate the club on how to properly accommodate the player while participating in club activities. Failure to do so is an assumption of risk and the club shall not be held responsible for any injury no matter how minor or major.

By reading this document, participants acknowledge and admit that there are inherent risks involved in the types of activities offered by NJGW. Therefore the participant accepts full financial responsibility for any injury that may occur either to the player or to any other player due to his/her negligence. Should the above-mentioned parties, or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreement, you agree to reimburse them for such fees and costs. Participants and players further agree to indemnify and hold harmless NJGW, their principals, agents, employees, and volunteers from liability for the injury or death of any person(s) and damage to property that may result from the participant’s negligent or intentional act or omission while participating in activities offered by NJGW.

By reading this document, you agree to have fully read and understand the foregoing assumption of risk, and release of liability and understand that by reading it, it obligates you to indemnify the parties named for any liability for injury or death of any person and damage to property caused by your negligent or intentional act or omission. You understand that by signing up for any program, you agree to this form and are waiving valuable legal rights.

Participation confirms you have carefully read this Agreement and fully understand its contents. Participants are aware that this is a release and waiver of liability and read it knowingly, voluntarily, and of your own free will.

While we are continuing practices during a global pandemic we understand that a player’s health extends past what typically occurs on a soccer field. New Jersey Gray Wolves FC will endeavor to ensure that proper health measures are taken, voluntary disclosure forms as well as ensuring our players have masks while around other New Jersey Gray Wolves FC players and coaches. We asked that you ensure your player has not been exposed to COVID-19 either in being in a state or area that New Jersey recommends quarantining before entering the state or around a person diagnosed with COVID-19 before participating in New Jersey Gray Wolves FC activities. However, there is no perfect system to ensure the virus does not spread. Thus, if you choose to participate in any New Jersey Gray Wolves FC activity you do so waiving any liability or responsibility to New Jersey Gray Wolves FC or any of its staff. All forms of waiver that exist in this agreement shall extend to any COVID-19 related ailments.

    Media Release

    By reading this agreement, you grant New Jersey Gray Wolves FC permission to use your photograph, videos, image and likeness in any and all publications for New Jersey Gray Wolves FC. This includes but is not limited to website entries, any form of social media or print publications without payment or any other consideration in perpetuity. You authorize New Jersey Gray Wolves FC to record, edit, alter, copy, exhibit, publish or distribute collectively, use all photos, videos and images. Participation in New Jersey Gray Wolves FC waives the right to inspect or approve the finished product, including written or electronic copy, wherein your photo appears. Additionally, you waive any right to royalties or any form of compensation arising or related to the use of the photograph or video images. You hereby hold harmless and release and forever discharge New Jersey Gray Wolves FC from all claims, demands, and causes of action which you, your heirs, representatives, executors, administrators, or any other persons acting on your behalf or on behalf of your estate which may have or may have by reason of such use of this authorization.
